Statement of Purpose: As an integral part of the multidisciplinary team, the Clinical Nurse utilizes the nursing process to provide therapeutic, direct patient care to patients and their families in the Perinatal and Neonatal Intensive Care Unit / Special Care Nursery settings. Core Accountabilities: A. Standards of Practice: • Collects comprehensive data pertinent to the healthcare consumer’s health and/or the situation (assessment) • Analyzes the assessment data to determine and make judgment to the nursing diagnoses or the issues (diagnosis) • Identifies expected outcomes for a plan individualized to the healthcare consumer or situation (outcomes identification) • Develops a plan that prescribes strategies and alternatives to attain expected outcomes (planning) • Implements the identified plan through coordination of care delivery, employing strategies to promote health and a safe environment (implementation) • Evaluates progress toward attainment of outcomes, reassesses as needed, and adjusts plan accordingly B. Demonstrates Competency in the Following Areas: • Knowledge of medications and their correct administration for the neonate based on his/her critical condition, age and weight • Follows the standard rights of medication administration and reduces the potential for medication errors. • Ability to assess neonate at delivery and perform neonatal resuscitation • Ability to perform head-to-toe assessment on infants and adults and reassessments as per policy. • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates family regarding neonatal pain management. • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical / health care errors. • Supports and maintains a culture of safety and quality. • Ability to monitor hemodynamic status of neonate and correctly interpret the results • Ability to perform waived testing (point-of-care testing) per facility policy and procedures • Ability to interpret results of waived tests; take appropriate action on waived test results. • Ability to formulate an individualized plan of care, revise plan as indicated by neonate’s response to treatment and evaluate overall plan daily for effectiveness • Formulates a teaching plan based upon identified family learning needs and evaluates effectiveness of learning. • Demonstrates knowledge of cardiac monitoring, identifies dysrhythmias and treats appropriately based upon Neonatal Resuscitation protocol • Provides care to neonates requiring extensive fluid and electrolyte managment • Communicates appropriately and clearly to shift supervisor and/or relief charge nurse, co-workers and physicians • Consults other departments as appropriate to provide for an interdisciplinary approach to the neonate’s needs • Demonstrates an ability to assist physicians and other providers with procedures in the NICU • Coordinates and supervise patient care as necessary • Demonstrates the ability to be flexible, organized and function under stressful situations • Treats patients and their families with respect and dignity • Interacts professionally with patient / family and involves family in the formation of the plan of care • Maintains a good working relationship both within the department and with other departments • Documentation meets current standards and policies. • Identifies and addresses psychosocial needs of patient and family • Manages and operates equipment safely and correctly D.
