Responsible for all file unit functions that includes, but is not limited to, filing, retrieving, and delivery of medical records within specified timeframes. Creates permanent folders for patients’ encounters. Purges medical records in accordance with state record retention regulations. Identifies, separates, and files loose medical record documentation and forms in the correct patient record. Responds to internal and external requests for dictation status and transcribed reports. Assembles inpatient, outpatient, emergency room, and miscellaneous records in accordance with hospital defined procedures. Supports release of information activities by pulling, printing, and/or copying medical records. Provides general department support in the realm of HIT I functions to ensure continuity of workflow. Meets departmental set quality and productivity standards. Participates in the overall philosophy of creating and supporting a self-directed team that performs the work effort under minimal supervision.
ESSENTIAL JOB FUNCTIONS AND CORE RESPONSIBILITIES
- Adheres to hospital and departmental security and confidentiality policies and procedure.
- Assists physicians in the department with the Physician Portal and Chart Maxx processes.
- Assists with special projects or duties as assigned.
- Completes the “Tracking Form” by listing the batch number and date record/documents were scanned 100% of the time.
- Demonstrates an ongoing understanding of and actively participates in performance improvement and cost containment programs.
- Ensures “Scanned Indicator Sheet” is present and scanned.
- Follow the scanner maintenance guide by cleaning scanners on the daily and monthly schedules.
- Maintains appropriate set up for scanner to ensure optimal image results.
- Maintains medical record shelves accurately, systematically, and orderly.
- Monitors receipt of records and follows-up with patient care areas if records are not returned to the department.
- Orders and processes records coming from and going to offsite storage.
- Organizes loose documents and forms.
- Places completed batches in appropriate area for indexing.
- Prepares fetal monitor strips for storage.
- Produces daily productivity reports outlining work processed.
- Provides departmental support as necessary. Areas of coverage include distribution of transcribed reports, front desk functions, all file unit functions, and chart completion functions within the realm of HIT I.
- Reads business emails at least twice a day.
- Responds to emergent and urgent release of medical information requests.
- Retrieves medical records from patient care areas after the patient is discharged.
- Retrieves paper medical records on request for authorized internal and external customers.
- Retrieves records from scanning shelf and performs scanning process according to procedure, keeping the shelf current to within 24 hours of receipt of record in the department 98% of
- the time.
- Supports release of information activities by pulling, printing, and/or copying medical
- records.
- Tapes torn pages, EKG strips, etc.
- Verifies that each batch is prepared appropriately, removing documents that cannot be scanned and removing staples, paperclips, etc.
Job Requirements Education Minimum (Required) High School Diploma or equivalent preferred Work Experience Minimum (Required) • Must be customer oriented and possess excellent communication and organizational skills. • Must be self-motivated, detail oriented and be able to work independently. • Previous hospital, clinic, or physician office experience preferred but not required. • Requires ability to problem solve, work under pressure in a positive, team manner, with diplomacy and tact. Preferred (Not required) Mandarin and/or Cantonese speaking preferred. Pay Transparency The hourly rate range for this position is $21.35 - $27.89.
