Clinical Staff Pharmacist - Price Care

The USC Alfred E. Mann School of Pharmacy is seeking a full-time Clinical Pharmacist. This individual will provide clinical pharmacy services through community pharmacies located in the Gateway Cities of Los Angeles, and be pivotal in advancing opportunities by developing and scaling up innovative and self-sustaining clinical pharmacy programs supported by existing policy, legislation, programs, and collaborations.
Key Responsibilities:
Medication Management: Conduct medication reconciliation, optimize drug regimens through collaborative practice, and educate patients on pharmacotherapy.
Clinical Collaboration: Work with multidisciplinary teams to deliver safe, comprehensive, and cost-effective drug therapy including referrals between care team members
Therapeutic Focus: Provide and/or lead clinical programs targeting high-risk general and specialty conditions and medications such as such as anticoagulation, tobacco cessation, obstructive airway disorders (e.g., asthma, COPD), cardiometabolic conditions including diabetes and hypertension, heart failure, mental health and substance use, pain management, etc.
Program Development: Lead initiatives to fully leverage all available resources (e.g., policy, legislation, value-based initiatives) to develop new clinical pharmacy programs that deliver high value while generating revenue, ensuring sustainability.
Liaison Role: Serve as a drug information resource for healthcare providers including physicians, nurses, and other pharmacists.
Documentation: Record patient interactions and interventions in electronic health records and other databases necessary for monitoring progress, conducting quality improvement, driving pay for performance measures, and reporting outcomes demonstrating impact / value of clinical programs.
Education and Training: Provide educational programs for healthcare professionals, and precept pharmacy students and residents.
Policy Development: Assist in creating policies and procedures to ensure safe medication use, clinical service workflow, storage, and security, aligning with regulatory compliance.
Additional Duties:
- Review medication profiles of patients seen in outpatient clinics.
- Identify and address barriers to medication adherence, implementing measures to facilitate access to medications.
- Collaborate with physicians to coordinate care delivery and optimize patient health outcomes.
Required Qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
- Active pharmacist license in the state of California.
- Strong clinical knowledge and expertise in medication therapy management.
- Excellent communication and interpersonal skills.
- Ability to work effectively in interdisciplinary teams and adapt to changing environments.
- Experience leveraging opportunities and developing successful, self-sustaining clinical programs.
- Commitment to patient-centered care and continuous professional development.
- Proficient in basic Microsoft Office applications including Word, Excel, PowerPoint
Preferred Qualifications:
- Completion of a pharmacy residency program
- Previous experience in providing and documenting medication management services
- Previous experience in precepting and teaching APPE/IPPE students/intern pharmacists, pharmacy technicians and pharmacy residents
- Multilingual, particularly Spanish
- Certificates: Medication Therapy Management, Immunization, Travel Health and Smoking cessation.
The hourly rate range for this position is $68.00 - $75.00. When extending an offer of employment, the University of Southern California considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate’s work experience, education/training, key skills, internal peer equity, federal, state, and local laws, contractual stipulations, grant funding, as well as external market and organizational considerations.
Minimum Education: Bachelor's degree Minimum Experience: 1 year Minimum Field of Expertise: Degree from accredited (ACPE) school of pharmacy. Registered pharmacist in state of California. Hospital pharmacy experience. Familiarity with unit dose, IV admixtures and clinical pharmacy practice.
