Healthcare Navigator

The Division of Primary Care and Department of Neurology at Keck School of Medicine of USC are seeking a Health Care Navigator to join our team of experts to improve care of older adults with newly identified memory and other cognitive concerns.  This “Brain Health Navigator” is a new, patient-facing role at Keck, designed to ensure that patients, families, and clinicians receive timely evaluation for newly identified memory and related cognitive concerns, while ensuring continuity and coordination of care.

Responsibilities include helping to develop an effective workflow; managing communications with patients, families, clinicians, and other staff; scheduling meetings, conducting assessments, preparing reports, and assisting in data collection for program evaluation.

This is a full-time hybrid position. 

Duties may include but are not limited to:

1.            Help design and monitor processes to meet needs of patients/families/clinicians and staff, with an emphasis on efficient communication.

2.            Coordinate communication between primary care (Family Medicine and Internal Medicine) and specialty care (Neurology)to support smooth patient care transitions across departments.

3.            Actively seek and use feedback from the patients, families/caregivers, clinicians, and staff to make improvements in the work process.

4.            Maintain close follow up with patients and remind/update clinicians on patient status/condition throughout the diagnostic process.

5.            Assist in planning and collecting data to meet project reporting goals.

6.            Keep track of patient care flows, to identify and monitor bottlenecks and time-to-completion of the diagnostic process. Regularly report results to supervisor. 

7.            Implement patient assessments necessary to facilitate timely diagnostic completion and establish a baseline for ongoing care planning.

8.            Arrange and attend meetings and trainings relevant to the work.

The hourly rate range for this position is $35.77 - $40.24. Minimum Education: Bachelor's degree Addtional Education Requirements Combined experience/education as substitute for minimum education Minimum Experience: 2 years Minimum Skills: Experience providing medical/clinical guidance. Advanced judgment, analytical, and decision-making skills, able to work independently with minimal supervision. Data gathering and analysis skills. Excellent time management skills. Demonstrated interpersonal, negotiation and problem-solving skills. Experience in customer service. Practiced use of discretion and decision-making ability. Excellent written and oral communication skills. Proven ability to build and maintain deep and meaningful relationships with community members and outside agencies. Preferred Education: Related undergraduate study And Specialized/technical training Preferred Certifications: BLS/CPR Certifications in CPR, basic life support and/or mental health first aid. Preferred Experience: 2 years Preferred Skills: Demonstrated experience in community healthcare and/or street medicine. A current understanding and appreciation of the cultural vibrancy and historical significance of local communities in Los Angeles and Southern California. Extensive knowledge and/or understanding of numerous cultural issues (e.g., immigration, socioeconomic, LGBTQIA+). Fluent in one or more languages in addition to English (e.g., Spanish, Korean, American Sign Language).
