Transplant Coordinator III, NP – Heart Acquisition – Full Time 8 Hour Days (Exempt) (Union)

The Transplant Coordinator III-NP, under the direction of a collaborating physician, assists in the care of patients with a variety of end stage organ diseases. Coordinates and participates in the medical/surgical management in either inpatient and/or outpatient setting of transplant and/or end stage organ diseases. Performs comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient's responses. Order medications, diagnostic studies, and therapeutic procedures related to their illnesses and track results timely as outlined. The Transplant Coordinator III-NP also assists other personnel to ensure high quality delivery of patient care and services to a transplant/end stage organ disease patient population. Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community. Participates in collaborative, interdisciplinary research activities related to transplant patients, process improvement or evidenced based practice activities. Attend professional meetings and conferences, maintain professional affiliations and enhance professional development to maintain knowledge of advances of transplant populations and end-stage organ diseases. Performs other special projects and duties as assigned. This position may require rotating on-call responsibilities as needed, including nights, weekends and holidays.

The Transplant Coordinator III-NP (TC III) will be responsible for the care of heart transplant patients in Los Angeles and may be responsible for managing alongside a supervising physician for post-transplant patients for the Las Vegas transplant clinic. The TC III may be required to attend the Las Vegas clinic alongside heart transplant physicians and engage in outreach to referring cardiologists in Las Vegas to boost referrals from Las Vegas clinic.

Essential Duties:

  • Coordinates and participates in the medical and surgical management in either inpatient or outpatient setting of transplant and/or end stage organ disease populations.
  • Performs comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient’s responses.
  • Furnishes/orders medications, diagnostic studies, and therapeutic procedures related to their illnesses and track results timely as outlined.
  • Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.
  • Participates in collaborative, interdisciplinary research activities related to transplant patients, process improvement or evidenced based practice activities.
  • Attend professional meetings and conferences, maintain professional affiliations and enhance professional development to maintain knowledge of advances of transplant populations and end-stage organ diseases.
  • May be required to be on call 24 hours a day and will participate in a call rotation, which includes relaying information to physician staff and facilitate the ongoing care of the patient as well as handling organ offers.
  • Performs other special projects and duties as assigned.

Required Qualifications:

  • Req Master's degree OR
  • Req Doctorate Degree in Nursing Practice Nurse Practitioner Program
  • Req 2 years Experience as an Acute Care Nurse Practitioner with focus in Cardiology, Advanced Heart Failure, and/or Cardiothoracic Surgery
  • Req Must have excellent communication skills, including the ability to speak, read and write English proficiently.
  • Req Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures.


Preferred Qualifications:

  • Pref 2 years Two years Transplant Coordination experience required and for, Heart Transplant – two years of cardiology experience Lung Transplant – two years of pulmonary experience Liver Transplant – two years of abdominal transplant experience Kidney / Pancreas Transplant – two years of abdominal transplant experience OR
  • Pref 1 year At least one year of NP experience preferred.


Required Licenses/Certifications:

  • Req Registered Nurse - RN (CA Board of Registered Nursing)
  • Req Acute Care Nurse Practitioner - NP (CA DCA) *Only if practice setting is in-patient, board certification as Acute Care NP.
  • Req Nurse Practitioner Furnisher (CA DCA)
  • Req DEA Certificate
  • Req National Provider Identifier (NPI)
  • Req Basic Life Support (BLS) Healthcare Provider from American Heart Association
  • Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)
  • Req Certification as a Nurse Practitioner through the American Association of Nurse Practitioners or American Nurse Credentialing Center.
  • Req Nurse Practitioner - Nevada State Board If no license upon hire, must be obtained within 3 months of hire or transfer date into position.

The annual base salary range for this position is $145,600.00 - $237,120.00.
