Patient Navigator

Pediatrics/County Maternal Child AIDS Los Angeles, California

Willing to make a difference in someone’s life?  You can make that happen by sending your application to become a “Patient Navigator” at the Pediatrics/Los Angeles County Maternal Child Program, a division of the Keck School of Medicine of USC, Department of Pediatrics.  Supported by a sub-award through AIDS United, the “Re-Engagement of HIV-Positive Women of Color” intervention is designed to best serve Women of Color (WoC) who are newly diagnosed with HIV or who have fallen out of HIV primary care.

This is a 5-year grant beginning in 2015 and ending 2020.  The position is full-time and pays $46,000-$48,000.

If selected, you will:

  • Be integrated into our clinical team serving as an advocate and liaison between the clinic medical provider and other clinic staff.
  • Be trained in Motivational Interviewing to create a welcoming environment to encourage client examination of self-motivations and engagement in care.
  • Link and re-engage Women of Color in HIV primary care.
  • Provide coaching and emotional support to patients who may need assistance in managing medical and case management appointments.
  • Work closely with case managers and the clinical team to better provide individualized patient-centered services over a short time period to address immediate patient needs and build trust between the patient and the clinic team.

Preferred experience:

  • Working with our target populations of HIV+ Women of Color.
  • Spanish speaking
  • Experience in clinical research and community outreach
